Abstract

Although multimodal traditional Chinese medicine external therapies demonstrate distinct utility for chronic musculoskeletal pain, therapeutic durability remains compromised by non-standardized execution and post-discharge compliance deficits. In this assessor-blinded randomized trial involving 186 patients, we investigated a stepped pathway combining standardized acupuncture, manual manipulation, and chronobiological moxibustion with an interactive digital health-management application, amidst iterative operational recalibrations to accommodate real-world tracking friction. Participants receiving digitally integrated care exhibited marked elevations in pressure pain thresholds alongside sustained reductions in serum substance P and calcitonin gene-related peptide compared to standardized care alone. However, these physiological adaptations may to some extent reflect behavioral reinforcement and structured self-surveillance rather than purely intrinsic bioelectrical recalibration across meridian networks. While this synthesis appears capable of mitigating twelve-week post-intervention recrudescence, subtle reporting biases within electronic logs cannot be discounted. Considering these interwoven factors, embedding codified external modalities within responsive digital architectures offers a promising framework for stabilizing musculoskeletal rehabilitation trajectories, though clarifying the precise neuroendocrine-behavioral interplay across diverse demographics indicates that further research is needed.
